Ngā Whakamārama- Service Description.
The programme consists of the following:
- Information about the process of the Annual Review, the diabetes tests and examination
- Transport and/or support to other services in preparation for your Annual Review and to your Annual Review
- Support with referrals to other services for whānau of people with diabetes, to gain information and education about this

Whānau who have been referred to our service by their GP/Doctor or practice nurse
Our service cover areas are: Hutt Valley, Lower Hutt, Petone, Stokes Valley, Upper Hutt, Wainuiomata and Western Hills.
Why have you been referred to our service?
There are many reasons why whānau are referred to ur service. Most common reasons:
- Inability to engage with/or contact whānau, due to their Annual Review
- Barriers to accessing their Annual Review e.g transport, support, lack of understanding the importance of heir Annual Review.
