Ngā Whakamārama- Service Description

Well Homes is tasked with reducing household crowding for whanau at risk of rheumatic fever in the Wellington region.  Collectively 850 Wellington families and whanau will be worked with over the next 2 years.

Well Homes is a pathway for nurses, doctors, social workers and community health workers to refer a family or whanau, who may be experiencing housing problems, for support.

Although eligibility for the service is open to everyone, including private homes owners, and private and social housing rentals, the Well Homes service is focused on families and whanau at risk of rheumatic fever.

Whanau who do not fall within the Well Homes eligibility criteria will be referred on to other suitable alternative housing programmes including: Warm Fuzzies Wellington[email protected]  and Eco Design Advisors.
